3/19/2024 EnchantaVaughn Approved Behavior: During todayâs visit, Laylah appeared to be in a shifted mood, as evidenced by her irritable emotions and puzzled facial expressions. Laylah described a pattern of consistent misfortunate events that happened today. Laylah expresses feelings of bad luck due to certain things not going her way. Laylah verbalizes different complaints but utilizes her coping skills properly once she was in a more stable mindset and mother reports she shuts down for 10 minutes. Behavior: Laylah demonstrates ability to hold herself accountable and adapt to new challenges. Laylah was reminded to utilize relaxation techniques and breathe so that she could stay calm and focus in the session as mother reports Laylah shuts down and isolate herself from others within the home for about 30 minutes or days. Intervention: QMHPA addressed Laylahâs negative behavior right away. QMHPA allowed Laylah to individually process what occurred throughout the day. QMHPA validated Laylahâs feelings but explained the importance of utilizing her coping skills. QMHPA stated âYou have to learn to ignore others and not be so easily triggered.â QMHPA isolated Laylah from distractions to decrease disruptive behaviors and to reduce excessive talking. QMHPA explored Laylahâs capability to cope with her stressors on her own. Intervention: QMHPA verbally identified Laylah's impulsiveness and encouraged Laylah to use self-control strategies. QMHPA brainstormed strategies with Laylah and encourage her to implement them on a daily basis. QMHPA also provided mediation and relaxation techniques to maintain appropriate behaviors. QMHPA explained the importance of being on task and refraining from outside distractions. QMHPA reviewed and gave feedback regarding ADHD and its causes and affect toward Laylah well-being and academic functioning. Response: Laylah actively participated with QMHPA as they discussed and processed her feelings. Laylah communicated that it is often hard to remain positive at all times about herself when she doesnât always have the best things or necessities that she needs. Laylah discussed numerous situations where she had to adapt even when she didnât want to but was thankful for the changes. Laylah was receptive as QMHPA demonstrated better ways to function in the home setting. Response: Laylah displayed active behaviors in the second half of the session. Laylah engaged in self-reflection, discussing good and bad parts of her day. Laylah chose to get herself together and participate. Laylah communicated that she feels that she will be more successful, due to having a woman as a counselor. Mom expressed to the counselor that though Laylahâs behavior has improved she still has to remind Laylah about remaining consistent with her reactions and perceptions.
